A Career Exploration Program for Deaf and Hard-of-Hearing Students

Session I: July 11-16, 2026
Session II: July 18-23, 2026
Session III: July 12-24, 2025

Please complete and submit this application.  We must receive the following required information before we can consider your application:

·         * A copy of your high school transcript (from ninth grade to present)

·         * A copy of your unaided audiogram (results without a hearing aid or cochlear implant)

·         * An official copy of your ACT/SAT/PSAT scores (optional)

·        *  Medical History (EYF Medical Form - Required Doctor Signatures)
