Please complete and submit this application. We must receive the following required information before we can consider your application.

A copy of your official high school transcript (from ninth grade to present)
A copy of your unaided audiogram (results without a hearing aid or cochlear implant)

Please Note:

Participants must be deaf/hard-of-hearing high school students residing in Illinois, aged 14-19 as of August 1, 2025.
Participation in the program does not guarantee eligibility for admission to RIT/NTID
